At Homegrown Counseling, we are committed to providing ethical, transparent, and respectful care. This page outlines important information regarding your rights as a client, including how to access your records, contact the appropriate licensing authority, and file a complaint if needed.

Access to Your Healthcare Records

Licensing & Regulatory Authority

You have the right to request access to your healthcare records in accordance with applicable laws.

To request your records:

  • Call our office at 817-961-1553

  • If you are requesting your own records, they can be released through the client portal

  • If records are being requested by a third party (such as a doctor, attorney, or for disability purposes), a signed Release of Information form is required. Once received, records may be securely faxed to the appropriate party

Additional documentation or identity verification may be required before records are released.


Homegrown Counseling providers include Licensed Professional Counselors (LPCs) and Licensed Clinical Social Workers (LCSWs), regulated by the Texas Behavioral Health Executive Council.

Filing a Complaint

Clients have the right to file a complaint regarding services received.
